Hope employs a Social Services Advocate who can help connect you to valuable resources in our local community. Please use the information below to reach out directly if you are in need of assistance.

Social Services Advocacy provides resources and networking within the community for the following:

  • Skilled home health services
  • Physicians
  • Nursing homes 
  • Assisted living facilities
  • Assessments for transitional care
  • Elder Affairs and Guardianship Legal Counselors
  • Health Symposiums
  • Durable Medical Equipment
  • In-home services
  • Assistance with SSDI/SSI applications
  • Financial hardship referrals 
  • Child Abuse Prevention and Advocacy
  • Substance Abuse Treatment referrals
  • BP Screening
